Metabolic Surgery: Where We Are

Abstract
Bariatric surgery has achieved the goal to control the obesity and its comorbidities in severely obese people. Among the comorbidities, type 2 diabetes (T2DM) is rearing up one of the biggest problem in global health and bariatric surgery has been well known as the most effective treatment option of T2DM in severely obese patients. Recently, it has been brought up that bariatric surgery can be applied to the patient with less obese patients for the control of T2DM. Till now, the mechanism to control T2DM by bariatric surgery has not been clearly explained. But, many observations have been reported and they proposed many hypothesis. In such a circumference, International Diabetes Federation (IDF) stated new position for management of T2DM which includes surgical treatment in less obese patients, after all. Especially, they mentioned for the Asian, that surgical treatment can be eligible for the patients with BMI over 27.5 kg/m2. In Korea, some surgeons have struggled in and caught up this new field. They studied and proposed the indications of metabolic surgery and observed and reported that insulin secretory function can be improved after metabolic surgery, and so on. Metabolic surgery is expected that it can control T2DM of less obese patients as well as normal weight patients even if so many studies and effort is needed.
